Another great arc but I liked Onikakushi a bit more. That last scene with Mion is just.. wow. When I saw it, I rewatched it a couple more times becuase it was so awesome.

The Shimon that attacked Keiichi at the hospital was an hallucination. He died of an acute heart failure.

Ah, that explains it. Also, with the information we know now about Hinamizawa syndrome, its very likely.

It may have been a hallucination but it was still pretty damn awesome for an ending.
First time we even heard about Mion having a twin sister. I think this was the next gruesome thing I ever saw. Rika's stabbing looked way, way brutal. Quite the intro.

In any case it was a great arc though I was definitely confused with who the real villain was. I mean there was just times that you really thought that it wasn't Mion at all, but then you definitely thought it was.

I love the ending of this arc. Higurashi is really a crazy show. :awesome

The hallucination is never stated. You just put 2 and 2 together. We know from Meakashi that Mion and Shion were definitely dead and we know Keiichi has HS which can cause hallucinations.
The heart failure was stated in the credit roll at the end of Meakashi.

When I first watched this arc, I thought that Shion wasn't real. Something like being Keiichi being delusional over some busty, green-haired classmate of his. :P
I had reserved my own thoughts on what the hell was happening, only to be refuted in its corresponding answer arc.
Seriously, Watanagashi-hen is also a WTF-fest. I have to give credit to Satsuki Yukino for delivering the goods in this arc. Her superb voice acting made this arc so damn great. I mean, come on, who wasn't creeped out with that crazy laughter during the phone call? :awesome

It may have been creepy, but it was a damn good ending for that episode.
