Location: The center of Maui Island on an alternate Earth where humans never existed.

Both are bloodlusted, neither get prep time, neither have any advance knowledge of the other's abilities. Battle to the KO or death.

Extremis should allow Iron Man to keep up with Gear 2, and flight allows him to keep out of range of Luffy's strength. There's also the fact that he can go underwater.

As long as Iron Man doesn't go hand to hand in the beginning he should be able to win. Luffy's body may be durable but it's not standing up to unibeams or full-power repulsor blasts, or god forbid, kinetic pulses.

I honestly don't think that Luffy would reach Stark wearing Extremis without getting some reaction from Tony.

if Sentry cant break through that armour, I doubt Luffy can.
Tony on the other hand can just blast Luffy's head off with his repulsor beam.

Luffy wouldn't even see IM. If he's bloodlusted, IM would just drop into visual stealth mode (invisible to the naked eye) immediately, fly up to 1000 feet and vaporize the island with the Uni-beam. It would be over in seconds.
