Both are huge, strong, and utter best.
Just that one is a complete berserker and the other is a skilled fighter.

Raoh from Fist of the North Star?
Well I heard he can make people explode with his attacks, but then again, Hulk can regenerate, so this could be tough.

Both are pretty ridiculous, but the Hulk just outclasses him too much in strength and durability. Hulk has punched a meteor twice the size of Earth in half, Roah is nowhere near that.

Depends on if Hulk can survive having his head explode. Roah can do this, he will do this, there is nothing Hulk can do about it.
Roah is faster than Hulk, pressure points work on Hulk (as prove by Capt. America), and Hulk doesn't even care about dodging.

But, has he ever survived something like that? He's come back from a skeleton, but his brain was still intact.
Maestro doesn't seem to think he could survive it, as suggested by the BFG designed to kill him.

I've read that one, he was eaten by bugs, but it didn't say he was completely eaten, only partially.

All of his organs and everything were pretty much ripped apart so that he was just a perforated shell of flesh left, and he still regenerated.

Still, even though his eyes were destroyed, his head mostly stayed intact, and for all we know, his brain. Which cannot really be proved, or misproved.
Even still, he a lot of mass to reconstitute from, Roah can make his entire body explode, or at least, Kenshiro could, I believe Roah knows most of the moves Kenshiro does, even the ultimate one.

Not all of them, only part of the intestines were eaten, his chest cavity is largely intact, and his skull doesn't allow you to see inside.

I'm not sure how different the manga is, but in the anime he defeats Ken.
