I'm feeling a wee bit silly

Take getting a good kick in the nuts. Now take that pain and turn it into a constant 24 hours of it.

So my question 24hours of a boot to the nuts pain vs anything other than death.

I'm stumped

Replace salt with Bullet Ants. They have the most painful sting in the world.

And inject them with Playtipus venom, the most painful venom know to man. Even Morphine can't help you
